Birth Plan Template (one-page) Digital Download
Experience shows that having a single-page Birth Plan to hand to your Birth Team at Admissions (and hang a copy on your room door) is more likely to be reviewed and respected by your care team.
This no-frills, beautifully designed Birth Plan Template goes over your most important needs and birth desires. Print it out, use a black pen to fill it out, and I recommend reviewing it with your chosen Care Provider during pregnancy and again upon Admission at your chosen birth location (most suitable for hospital or birth center).
All options shown on the birth plan can (and should) be researched beforehand and/or discussed in depth with your Care Provider during pregnancy to help you make an informed choice. This also helps you be able to go into your birth feeling knowledgeable and empowered to know what is best for you and your baby, as well as be firm in turning down or open to considering alternatives instead of being unsure because you don't know what you're really being offered by your Care Team in the middle of labor!

Printing Instructions:
The Birth Plan Template is formatted for standard 8.5 x 11 inch printer paper.
Step 1: After you download and open your file, select Print and open up your printing settings.
Step 2: Select "Pages - Custom - 2" to avoid printing the first "How to use This Template" page.
Step 3: Select the number of Copies you want to print (recommend at least two, after your test print!)
Step 4: Then make sure your print Scale is set to either "fit to paper", "fit to printable area", or Custom -- select % // whichever printing scale that best suits your home printer without cutting off any areas of the template-- not all of them will print within the same space depending on your printer, so you may need to do a test print. For best quality, print in full color if possible.
Step 5: Fill out your birth plan copies, being sure to research any procedures you may be unsure of or need more information on before deciding on whether you would like it or not. Keep one copy with you/in your birth room (sometimes you can hang them up on the door) and give another copy to your birth team lead/head nurse when you check-in for labor.
